    How does organ donation and transplantation work?

    Organ donation and relocation is a complex process. It begins with donation and ends with two surgeries to transfer an organ from one person to another. The intermediate steps are completed in a very short timeframe, often in a single day.

    • Donor identification and selection
    • Obtaining consent (donation)
    • Matching donors and recipients
    • Transplant coordination
    • Organ retrieval
    • Organ transplant
    • Donor identification

    Most organ donors have recently died without organ damage. Often, this was due to a traumatic brain injury that caused brainstem death or sudden cardiac arrest. In these cases, hospitals can preserve organs after the patient’s death through mechanical ventilation. In the United States, medical professionals from the Organ Procurement and Transplantation Network (OPTN) assess the patient’s health to determine if they are a good candidate for organ donation.

    Obtaining Consent (Donation)

    Many organ donors decide to donate their organs before they die. OPTN maintains a national database of registered organ donors. Once eligibility is confirmed, OPTN staff check to see if the potential donor is registered. If so, they will notify the donor’s family of their consent to donate. If the donor is not registered, OPTN will consult with the family about the possibility of donating. At this point, the family can decide to donate their organs themselves.

    Donor and Recipient Matching

    The next step is to match the donor’s organs and tissues with those in need. This process is largely automated. OPTN enters information about the donor’s blood type, body size, and available organs and tissues into a computer system. The system finds the most suitable organs from its database of potential recipients. First, it matches organs with potential recipients based on their physical characteristics. It then ranks potential recipients based on their need and, finally, their location.

    Transplant Coordination

    Once the recipient is identified, the transplant center will receive an email notification with the offer of a donor organ. The transplant team will make the final decision on accepting the organ for the patient. If accepted, they will coordinate with the donor organization (PO). They will set up two operating rooms for the donor and recipient as soon as possible.
